To request for a new cheque book, download the PDF form below, print it out, fill up and sign to agree to the agreement. Then, send the form yourself to Country of OKITO (COO) via walk-in; or you can ship the form through EDL. You MUST use your handwriting.

Make sure the form code is this: QQ/CFD/BB/C-1/Jan-26

EDL shipping fees for sending request form and receiving new cheque will be covered by Basketball Bank. If you sent your form through EDL and would like to claim EDL shipping fees, please contact us via email at qqcarecentre@gmail.com (Attn: Basketball Bank).
